Step 1

• Double click the mouse button. It will display a dialog box in the window.

Step 2

• You will see a dialog box appear as the picture shown.

1. Types a descriptive name for the checkbox.
   

2. Type a value for the checkbox.
   

3. Click Checked if you want an option to appear selected when the form first loads in the browser.
